New Members Joining Troop (checklist)
2012 Registration Fees: The Registration Fees for the upcoming Troop 109 year are $125.00 per Scout. Please bring a check made out to BSA Troop 109 to the Registration Meeting (Sunday, January 22nd) along with the following completed paperwork:
- Registration Form
- Troop 109 Policies & Procedures
- Motor Vehicle Information Form
- Parental Consent & Medication Authorization Form- Please have this form Notarized before handing it in at the Registration meeting.
- Annual Health and Medical Record (Parts A, B & C required)
- Webelos Cass B Order Form - only ordered at registration time
- Photo Release Form
Returning Scouts (checklist)
2012 Re-Registration Fees: The Re-Registration Fees for the upcoming Troop 109 year are $75.00 per Scout. Please bring a check made out to BSA Troop 109 to the Registration Meeting (Tuesday, Janurary 17th) along with the following completed paperwork:
- Motor Vehicle Information Form
- Troop 109 Class B Order Form - only ordered at registration time
- Annual Health and Medical Record (Parts A, B & C required)
- Photo Release Form
- Parental Consent & Medication Authorization Form- Please have this form Notarized before handing it in at the Registration meeting.
Adult Leaders
- Adult Leader Application- for new Leadership positions only (not required each year, unless you are changing positions).
- Annual Health and Medical Record (Parts A, B & C required)
- Merit Badge Counselor Application
